Disclosure Regarding Background Checks

LulaLou's Ltd Liability Co., doing business as LulaRoost ("LulaRoost"), may obtain one or more consumer reports about you from a consumer reporting agency. It uses them to decide whether you may provide rides through LulaRoost as an independent driver.

If you're approved, LulaRoost may obtain additional consumer reports about you at any time while you drive with LulaRoost. This includes at least one annual check and continuous monitoring of your criminal and driving records.

These consumer reports may include:

  • criminal history records
  • sex offender registry records, including the National Sex Offender Public Website
  • your driving record (motor vehicle record), including license status, violations and accidents
  • verification of your identity, and of the addresses you have used

The consumer reporting agency is Checkr, Inc., 1 Montgomery Street, Suite 2400, San Francisco, CA 94104 (checkr.com). You can see your report and contact Checkr at candidate.checkr.com. LulaRoost may use another consumer reporting agency in the future and will tell you if it does.

You have rights under the federal Fair Credit Reporting Act, which are described in "A Summary of Your Rights Under the Fair Credit Reporting Act", available at consumerfinance.gov/learnmore. Before LulaRoost takes any action against you based in whole or in part on a consumer report, it will give you a copy of the report and that summary, and a chance to dispute anything in the report that is inaccurate or incomplete.

Authorization

I have read the Disclosure Regarding Background Checks above.

I authorize LulaRoost to obtain consumer reports about me from Checkr, Inc., or another consumer reporting agency LulaRoost uses:

  • in connection with my application to drive with LulaRoost
  • at any time while I'm a LulaRoost driver, including annual checks and continuous monitoring of my criminal and driving records

I authorize courts, state motor vehicle departments, law enforcement agencies, and other record keepers to give the consumer reporting agency the information it needs to prepare these reports.

I understand that I can ask the consumer reporting agency for a copy of any report about me, and dispute anything in it that is inaccurate or incomplete.

I agree that checking the box and typing my name below is my electronic signature and has the same effect as a handwritten signature.
